Looking for a DOT DMV Physical Exam, CDL Physical Exam, or a School Bus Driver Physical?

 

Our Practice has extensive knowledge of the DOT Physical exam guidelines, we offer exams for independent drivers and provides special pricing for both local and national motor carriers. Certified and trained by the FMCSA, our providers will ensure driver safety in compliance with all regulations. Under new Department of Transportation (DOT) guidelines, all medical examiners must be FMCSA certified.

 

What to Bring for Your DOT Physical Exam Preparation:

• Continue taking your prescribed medications as directed.

• Avoid using the restroom right before your appointment, as a small urine sample will be required for a kidney screen (this is not a drug test).

 

Items to Bring:

• A valid driver’s license.

• Eyeglasses or contact lenses (if applicable). Bringing a recent eye exam report is helpful but not required.

• A working hearing aid (if applicable).

• A list of medications and the contact information for the prescribing doctor.

• A medical release letter from your primary care doctor or specialist for any ongoing medical conditions, recent   illnesses, injuries, or surgeries.

• A current FMCSA Waiver or Exemption Certificate or Skills Performance Evaluation (if applicable).

• If you have sleep apnea, you are required to bring a CPAP compliance report for the last 30 days.

What to Expect During the Exam:

• The doctor will review your health history with you.

• A thorough physical examination will be performed.

• A urine screen (dip-stick test) will be conducted to check for potential disqualifying medical conditions such as protein, blood, or sugar in the urine.

• An Electrocardiogram

 

DOT Medical Card Validity:

• 2 years – If you meet all FMCSA DOT regulations for commercial driver fitness.

• A limited duration, typically 1 year or 6 months, if monitoring is required for certain medical conditions.

• A limited period, usually 1 to 3 months, if you have a newly diagnosed condition that you’re working with your primary care doctor to manage.
